Privacy Policy

The Prostate Cancer Center at Saint Joseph’s Hospital, Inc. (“PCC” “we” or “us”) recognizes that you care how we collect information on and through our web site, located at www.gaprostatecancer.org (the “Site”), and how we use and share that information. We have adopted this Privacy Policy to let you know how we collect information on and through the Site and how we use and disclose this information. This policy also contains information on the choices that are available to you regarding our collection and use of personally identifiable information (“PII”), and how you may access, review and change PII you provide through the Site. 

1.         Applicability of this Privacy Policy

This Privacy Policy applies only to information gathered through the Site and when you contact us in connection with your use of the Site. This Privacy Policy does not reflect our practices with respect to information gathered through other web sites that we or our affiliates operate or through off-line means. 

2.         Transfer of Information.

Please be aware the servers that make the Site available and that store the information that is collected through the Site are located in the United States of America (“USA”). If you are located outside the USA, please note that the information you provide will be transferred to the USA. The laws of the USA may not provide the same level of legal data protection as the country from which you access the Site. By using the Site and submitting PII to us, you consent to and authorize such transfers and the use and processing by us of information as described in this Privacy Policy.

a.         Information You Actively Provide:

You are not required to provide any PII to us in connection with your use of the Site. You should only provide information regarding yourself, however, if you submit any PII of any third party through the Site or in connection with your use of the Site, you represent and warrant you have the necessary approvals and permissions to provide that PII to us and for us to use and disclose that information in accordance with this Privacy Policy. 

b.         Automatically Collected Information - Use of Cookies/Web Beacons:

We may use cookies, web beacons and related technologies to automatically collect information regarding your use of the Site. Cookies are textual pieces of information that a web site server sends to your browser while you are viewing the web site. Cookies do not contain PII; however, once you choose to furnish us with PII, this information may be linked to the data stored in the cookie. Web beacons are small pieces of data that are embedded in images on the pages of web sites. We automatically collect several categories of information, including, without limitation, your IP address, information from server logs found on your browser, the type of web browser and operating system you are using, the name of the domain and host from which you access the Internet, the date and time you access parts of the Site, and the Internet address of the site from which you linked directly to the Site. 

If we send you an email, the email may include a “click-through URL” linked to a particular page on the Site. If you click on the URL, we may collect information related to the “click-through.” Please note: We will NEVER send you an email that (i) asks you to reply to the email with any financial account information, or (ii) contains an embedded link to a page that asks for any financial account information. Please contact us immediately if you ever receive an email requesting that type of information or that contains a link to a page that requests that type of information and the email appears to come from us. 

4.         How Information is Used.

a.         Personally Identifiable Information:

We use PII collected on or through the Site (i) to correspond with you regarding questions you may have about our services, (ii) to process registrations, (iii) upon your request, to provide you with information about the Site and our services, (iv) to help protect you, and us, against error and fraud, (v) for archival purposes, and (vi) to track your usage of the Site. 

b.         Automatically Collected Information:

We use automatically collected information to analyze and understand how the Site is used and to improve a user’s experience on the Site. We may use information gathered from cookies, web beacons and related technologies to, for example: (i) customize account pages for you, (ii) track user visits and account features, and (iii) determine what features and pages are most frequently requested or visited. 

In order to learn about where visitors to our Site come from and how they interact with the Site, we use third-party analytics tools. The third parties providing those analytics tools may retain and use, subject to the terms of the third party’s privacy policy, anonymous traffic data collected in our use of the analytics tools. For example, we may use Google Analytics and Google may retain and use, subject to the terms of the Google Privacy Policy (located at http://www.google.com/privacy.html, or such other URL as Google may provide from time to time), anonymous traffic data collected in our use of the Google Analytics tool. 

We may aggregate and use certain automatically collected information and non-personally identifiable information for our own research and marketing purposes. In addition, we may share, sell, rent, trade or publish that aggregate information, and statistical data derived from such information, with our marketing partners, advertisers or other third parties for their research and marketing purposes. 

5.         Disclosure of PII.

Except as otherwise provided in this Privacy Policy or with your consent, we will not share, disclose or sell to third parties PII you provide on or through this Site. We may disclose PII you provide to us with certain third parties that perform services on our behalf. The services provided by those third parties may include: operating the Site, hosting the Site, providing the information services you request through the Site, marketing and promotional material distribution, web site evaluation, data analysis and, where applicable, data cleansing. Those third parties are not authorized to use or disclose PII you provide to us on or through this Site for any purpose other than to perform the services designated by us. 

We may disclose PII to a third party for legal compliance purposes. For example, we may be required to disclose PII to respond to a law enforcement agency’s request, subpoena, court order or other legal process. We may also disclose this information where we believe it is necessary to investigate or address unlawful acts or acts that may endanger the health or safety of a consumer or the general public and to verify or enforce compliance with the Terms of Use and other terms and policies applicable to our Site. In addition, the PII you provide to us may be transferred as an asset in connection with a merger or sale involving all or part of ICRS or as part of a corporate reorganization, sale or other change of control. 

6.         Children and Privacy.

We offer our services and our Site for use by adults; we do not intend our Site to be used by children. If you are a parent or guardian of a child under the age of 13 and suspect he/she has provided PII on or through the Site, please contact us using the contact information set forth in this Privacy Policy. 

7.         Steps Taken To Keep PII Secure.

We employ various reasonable physical and electronic measures and managerial procedures to help protect the PII that we collect online. However, as no measures or procedures are completely effective against unauthorized access, use or disclosure, we cannot ensure or warrant the security of any PII. 

8.         Accessing, Correcting, Updating and Deleting PII.

You may submit requests to access, review, correct, update and delete PII that you have provided to us by calling or by sending an e-mail or letter to us using the contact information set forth in Section 12. Please clearly state your request with respect to the PII in your discussion/contact with us. 

We reserve the right to retain any PII reasonably necessary to appropriately document our business activities and for archival and record retention purposes. We will store PII for as long as reasonably necessary for the purposes described in this Privacy Policy.  

9.         Choices Regarding Receiving Promotional Communications and Disclosure of PII. 

When you call or send an email or letter to PCC using the contact information set forth in Section 12, you will have an opportunity to (i) make or change your selections regarding receiving promotional e-mail communications from us, and (ii) make or change your selections regarding our disclosure of PII relating to you to third parties for their marketing purposes. In addition, if you have opted-in to receive promotional e-mail communications from us, you will have an opportunity to opt-out of receiving those communications by clicking on an "unsubscribe" hyperlink contained in promotional e-mails we send you. Please note that “opting-out” will not stop marketing or advertising content that is generated for distribution or is otherwise queued for transmission to you prior to the time when we can implement your request. 

10.      Applicability of this Privacy Policy When You Link to Other Web sites.

The Site may provide links to or otherwise direct you to web sites owned or operated by third parties or other web sites made available by us or our affiliates to which this Privacy Policy does not apply (“Other Web sites”). This Privacy Policy does not reflect the information collection, use and disclosure practices of the Other Web sites or the operators or owners of Other Web sites. Your use of Other Web sites and the information you provide in connection with your use of those Other Web sites will be governed by the terms of use, privacy policies and other additional terms, notices and policies provided with those Other Web sites. To understand how the owners or operators of Other Web sites will collect, use, store and otherwise make information available to others, you should review the policies of those Other Web sites.
